Hi there, plans like this one require a little bit of detective work. It appears from a brief perusal of results on Google from looking up 'Susquehanna Pfaltzgraff', that the company is still in business. In fact, it appears it's a major holding company. I took things a step further and attempted to look up the plan's Form 5500. You can do this yourself at www.freeerisa.benefitspro.com. Register for a free account, and then search for Susquehanna Pfaltzgraff. The search will result in several employer plans the company administers. This is not unusual for such a large company - they have former pensions, 401(k)'s, and more. My guess, based on what you've mentioned, is that the plan you need may be the one named "Susquehanna Pfaltzgraff Co. Employee Savings Plan". This is a plan that is inactive, which may correspond to having been in place many years ago and now being a defunct plan. Don't worry - it doesn't mean your money is in jeopardy; it just means there aren't any current employees who are a part of the plan. The phone number listed on the Form 5500 filing is (717) 848-5500. I tried the number and it works. This is also the same phone number I find at this address, which gives a very basic overview of the company and provides its basic location and other info:
Hope this helps you out. I know tracking down these kinds of things can be confusing and time-consuming. When you call, be prepared that you might have to tell your story 3-4 times and be transferred around. My guess, though, is that this will ultimately get you to someone who can get you in touch with your plan, in order to reconnect with your account. Good luck!

If Susquehanna Pfaltzgraff is still in business, call them and ask.
Do you have an old statement of your 401k? There should be a phone number on it that you can call.
