Shayne, you should simply contact this employer and inquire. They should be able to give you all the information you need. Also, do not necessarily rollover this 401(k) asset to an IRA without first speaking to a good advisor because there are pros and cons to rolling it over, such as losing your ability to borrow up to 50% from it when necessary.
Many people forget to roll their 401(k)s into IRAs when they leave an employer. Then if the former company closes, these accounts are usually difficult to find. To track down a missing 401(k): contact the Department of Labor’s Employee Benefits Security Administration at 866-444-3272. Track down a lost pension go to http://www.pbgc.gov/documents/finding-a-lost-pension.pdf.
Shayne, call Janet in the H.R. Dept. at Ohio Valley Home Health and she can tell you the answer to that. Her number is 330-385-2333. She should be able to locate your records if you still have assets in the plan or tell you if you have a different status.
Janet should also be able to tell you who the recordkeeper is and who the investment firm, with their contact numbers.
If you should want to roll that money into an IRA rather than keeping it there, she can give you the forms to do that. You might also consider rolling the money into your retirement plan with your current employer, if that plan allows you to do that.
