Contact your HR department for both your current and former employers. One of them should be able to provide you the documents that you need.

In addition, we here at RFA are currently offering a free 401(k) tuneup at no cost for those who want a quick analysis and recommendations. All you have to do is email us at info@rfawealth.com or call in at 800-682-3237 and one of our experts will respond to your question immediately.

You should be receiving quarterly statements either online or paper depending on what was selected at enrollment. Have you moved or changed email address? Could the statements be in your email junk folder? If you don't find them there, contact your plan administrator to get your address updated.

Try your HR department. They should have the most recent information and/or know who to contact.
