Contact your HR department for both your current and former employers. One of them should be able to provide you the documents that you need.

You should be receiving quarterly statements either online or paper depending on what was selected at enrollment. Have you moved or changed email address? Could the statements be in your email junk folder? If you don't find them there, contact your plan administrator to get your address updated.
