How Can I appeal a 401k loan?

I believe it was 2012 when I had taken out a loan through my employers 401k. I went out on leave in 2014 (maternity leave) but with the birth of my child I was not able to make my last payment which was less than $20. The balance was taxed when I did my tax filing that year. Now it is 2017 and I urgently need a loan however I am told I am no longer eligible as the balance had to be taken out of my taxes and not paid directly. I spoke to HR and they advised I can try to file an appeal but no further information was provided on how I can do this? HR manager is no longer with the company and currently do not have the position fulfilled to gain more information.

Can anyone help on how I can file an appeal ?
